PARTICIPANT INFORMATION

Up to 4 children can be listed. If more, please contact [email protected]. PLEASE BE SURE TO LIST ALL PLAYERS EVEN IF YOU ARE NOT SURE THEY WILL BE PLAYING HOCKEY. THIS WILL PREVENT YOU FROM RETURNING TO THIS FORM TO FILL IT ALL OUT AGAIN.

Membership Agreement

I/My participating child hereby agree to abide by and support the current Ontario Minor Hockey Association Code of Conduct, and rules, regulations and decisions of the OMHA (Ontario Minor Hockey Association), OHF (Ontario Hockey Federation), HC (Hockey Canada) and OMA (Orangeville Minor Hockey). The OMHA Code of Conduct identifies the standard of behaviour, which is expected of all OMHA members and participants. As a member of the OMHA, for the purpose of this policy participants shall include all OMH players, parents, guardians, coaches, officials, volunteers, directors, officers, committee members, convenors, team managers, trainers, administrators and employees.


OMH is committed to providing an environment in which all individuals are treated with respect.

Members and participants of OMH shall conduct themselves at all times in a manner consistent with the values of the OMHA and OMH, which includes fairness, integrity and mutual respect. During the course of all OMHA and OMH activities and events, members shall avoid behaviour, which brings OMHA, OMH or the sport of hockey into disrepute, including but not limited to the use of alcohol, cannabis and non-medical drugs.

OMH members and participants shall at all times adhere to OMHA operational policies and procedures, rules and regulations governing OHF events and activities, to rules and regulations governing any competitions in which participants play on behalf of the OMHA and to the OMH By-Laws, rules of operation, policies and procedures.

I, as a member and participant of OMH, shall not engage in any activities or behaviours which interfere with a competition or with any player or team’s preparation for a competition or which endanger the safety of others.

I, as a member and participant of OMH, shall refrain from comments or behaviours, which are disrespectful, offensive, abusive, racist, or sexist. In particular, behaviour, which constitutes harassment or abuse, will not be tolerated and will be dealt with under OMHA’s Harassment Policy and OMH By-laws.

I, as a member or participant have read and understood the OMHA Social Media Policy. 

I, as a member or participant, acknowledge that from time to time, I/my child will be travelling from place to place, sleeping away from home, and eating away from home. I/my child may reserve the right to take action against a wrongdoer but, subject to that, I release the OMHA and OMH, including all members, coaches, managers, and directors, from any responsibility.

I hereby understand and appreciate that participation as a hockey player carries risks to me or my participating child of serious injury, including permanent disability, paralysis, or death. I/my participating child voluntarily and knowingly acknowledge, accept and assume these risks.

Failure to comply with this Member Agreement may result in disciplinary action in accordance with the Discipline Policy of the OMHA and OMH By-laws and Policies. Such action may result in the member losing the privileges that come with membership in OMH and OMHA, including the opportunity to participate in OMH and OMHA activities and events, both present and future.

I the participant/parent have become subject to the rules, regulations and decisions of the OMHA, OHF, HC and OMH and its Board of Directors and agree to abide by such rules, regulations and decisions of the OMHA, OHF, HC and OMH. I am aware that these rules and regulations are available to me through my local association and certify that the provided information is true.

I signify my agreement to the above terms and conditions when I register my child.

Ontario Hockey Federation Rowan’s Law Acknowledgement Form

The Ontario Government has enacted Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1 (“Act”). Ontario Regulation 161/19, the Act requires all sport organizations as defined in the Regulation (“Sports Organization”), which includes the Ontario Hockey Federation (“OHF”), to have a Concussion Code of Conduct. This Concussion Code of Conduct must require participants, as set out in the Act, to review the Ontario Government’s issued Concussion Awareness Resources on an annual basis. 

A participant is subject to a Concussion Code of Conduct for each Sports Organization a participant registers with.

The OHF Concussion Code of Conduct and the appropriate Concussion Awareness Resources must be reviewed before you can register/participate in the OHF.

MEDICAL FORM

As with all seasons, all players/participants shall have an updated medical form filled out for the 2020-2021 Season.

Filling out this form now will greatly assist us in being proactive for rostering of the season, especially for House League Players.  Please fill out the form and:

1. For Rep/MD players: keep to hand in during parent meetings.
2. For House League players, please fill out and hand into the teams trainer once the season starts.
